Paediatric Forms

Here at C4K we believe if your goal is to have your practice full of children then your clinic, your systems and procedures, in fact everything you do, must be aligned with and it must support your vision.

For this reason we believe you simply must have a separate history form and patient card for children to that which you use for adults. Kids are different to adults. You need to know different information. At each stage of a child’s life there is important age specific information that is unique to that particular period of growth and development for that child. At C4K we have therefore produced a range of paediatric patient forms which allow you to address this information during your case history.

Please note that due to the high cost of postage, the forms are only available for Australian Chiropractors.

We have a history form which is filled in by the parent at their child’s first appointment with you.

We also have three age specific patient cards which are for the chiropractor to fill in during that first visit and at subsequent appointments.
